Your Agent is the person you have legally designated to make decisions about your health care if you are unable to speak for yourself. They are your legally recognized representative in the hospital.

Designating a health care agent is the only legally binding aspect of an advance health care directive (AHCD). A good choice for a health care agent is someone who meets at least three of the following criteria: 1) knows you well; 2)  maintains an open line of communication with you; 3) respects your wishes and values even if they differ from their own; 4) lives close by; 5) is comfortable in a medical setting; 6) is in good health; 7) is diplomatic; 8) has a good relationship with other members of your family.
